    What is Senior Outreach?

  • The Senior Outreach Program provides prevention, education, and referrals typically with

  • a focus on substance abuse. The Senior Outreach Program can assist professionals and others to recognize signs and symptoms of possible substance abuse in the elderly population.

    Who is the Senior Outreach Program for?
  • Males or Females aged 55 and older.
  • Those who reside in Hartford and surrounding area.
  • Seniors currently residing in any setting including residential treatment, senior housing, shelter, etc.

    What is required of individuals who need services?
  • Willingness to engage in services
  • Reside in Hartford or nearby cities

    What does Senior Outreach Services provide?
  • Initial meeting to determine the needs of the prospective resident.
  • Individualized treatment planning
  • Referrals
  • Individual, family, group and support groups focusing on the substance abuse and other life issues
  • Community outreach to local agencies and families
