The Senior Outreach Program provides prevention, education, and referrals typically with
a focus on substance abuse. The Senior Outreach Program can assist professionals and
others to recognize signs and symptoms of possible substance abuse in the elderly
population.
What is Senior Outreach?
- Males or Females aged 55 and older.
- Those who reside in Hartford and surrounding area.
- Seniors currently residing in any setting including residential treatment, senior housing, shelter, etc.
- Willingness to engage in services
- Reside in Hartford or nearby cities
- Initial meeting to determine the needs of the prospective resident.
- Individualized treatment planning
- Referrals
- Individual, family, group and support groups focusing on the substance abuse and other life issues
- Community outreach to local agencies and families
- Coping skills
- Relapse prevention skills
- Referrals to appropriate community resources
- Case management services
